Update: Treatment for Leptospirosis


Indication
Preferred agent (Adult dosing)

Mild-moderate disease
Doxycycline
100mg PO q12h for 5-7 days1,2,3

Azithromycin 
500mg PO q24h for 3 days1

Amoxicillin 
500mg PO q8-12h2

Ampicillin
500-750mg PO q6h2
Severe disease
Benzylpenicillin 2MU IV q6h for 5-7 days1
Ceftriaxone 
1-2g IV q24h for 7 days1,3

Cefotaxime
1g IV q8h for 7 days1

Ampicillin
500-1000mg IV q6h2

Doxycycline
100 mg IV q12hfor 7 days3

Azithromycin
500 mg IV on day 1, followed by 250mg q24h2

Erythromycin
500-1000 mg IV q6h2
