Conversion of Carvedilol to Bisoprolol

  • Switch from carvedilol to bisoprolol in a 5:1 conversion
  • Summary of conversion 
Carvedilol
Bisoprolol
3.125mg twice daily
1.25mg once daily
6.25mg twice daily
2.5mg once daily
12.5mg twice daily
5mg once daily
25mg twice daily (may titrate to 50mg twice daily if > 85 kg)
10mg once daily
